Introduction to the service plan for cardiovascular disease

The purpose of this assignment is to develop a population-based service plan using the topic of a chronic condition. Planning for a particular population group relates to planning for a health issue (or issues) for a particular population cohort. This type of planning may also target a particular geographic catchment (eg the State of New South Wales OR Western Sydney LHD etc).

Cardiovascular disease

1. Choose a population base for the plan. It can be a Local Health District or State or even a national plan and should relate to the population with the condition, or risk factors which may lead to onset of the condition.

2. Prepare an introductory section explaining the purpose of the Plan and justification the selection of the condition and population to be served.
