Field Practicum report: Building community capacity to achieve the human rights of all persons with disabilities

Report for Field Practicum

The Field Practicum report needs to be segregated in two components:
1. Process document
2. Final Detailed Report
The participants are expected to submit these reports at the end of their Field Practicum. Failure to
submit the report will impede the evaluation of the Field Practicum module.
Process document
A process document is a weekly summary report where the participants are expected to record their
weekly tasks and activities. The report should entail the methodology followed for the task assigned for
the field practicum. Time utilized and the progress made in each week should be documented in clear
and simple format. Constraints faced during fulfilling these tasks also need to be mentioned. In this
brief weekly report the participants will reflect on their assigned task and the progress on the objectives
of the field practicum.
Final Detailed Report
The final report should include the following :
1. Objectives: Specific task assigned with a work plan for 4 weeks.
2. Outcome: Deliverables
3. Summary of the process for task completion.
4. Further scope
5. Findings and suggestions to the organisation
6. Achievements: Elaboration of complete and incomplete task
